For your first research question you would like to do a correlation of two variables:
RQ1: How does team effectiveness correlate with “Leapfrog Hospital Safety Grades” [LHSGs] across healthcare systems?
Your variables are team effectiveness from a survey and Leapfrog Hospital Safety Grades which are letter grades. First we will need to take a closer look at your instrument to see how you will calculate effectiveness as a variable. Next the LHSGs are letters and not a continuous scale. For a correlation you will need to have two continuous variables to meet assumption 1 below.

First we must make sure that the calculation of effectiveness from your survey will lead to continuous variable. We can do this in a future meeting. Second you will need to find a score possibly the 5 Hospital Associated Infections score for each department to do a correlation. For any inferential statistics you need a sample of at least 30. To be certain you will have to do a GPower calculation to estimate the minimum sample size which can be around 68 for a correlation. We can work that out as well.
For research question 2 it is stated as a t test or an ANOVA to compare the two types of teams high versus low in terms of effectiveness.
